29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a,     On the west side of the second floor, there was an opening in the guardrail system where employees were framing walls thereby exposing employees to a 10'6" fall hazard.  b.     On the southwest side of the buidlng where employees were unloading materials, the guardrail system was removed and not replaced when the work was completed thereby exposing employees to a 10'6" fall hazard.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers.  a.     On the first floor, there were several floor holes varying in size from 23 inches across to 11 inches deep thereby exposing employees to a trip/fall hazard.

29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication program developed by the employer, including an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employees can obtain and use the appropriate hazard information.  a.     On the site where chemicals were used, the employer did not provide training under the new hazard communication program for labeling, and safety data sheets thereby exposing employees to a contact hazard.
