29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: On South side of building, on or about March 19, 2014 employees were exposed to a fall of greater than 15 feet while putting a new metal roof on a new construction building without a fall protection system in place.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a)On East side of building, on or about March 19, 2014, employees were exposed to a fall hazard of approximately 17 feet to a hard, compact surface below while trying to access an upper landing (roof) from a Werner aluminum ladder that did not extend 3 feet past the upper landing. b)On East side of building, on or about March 19, 2014, employees were exposed to a fall hazard of approximately 17 feet to a hard, compact surface below while trying to access an upper landing (roof) from a Werner fiberglass ladder that did not extend 3 feet past the upper landing.
