29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed: Drilling rig derrick: On or about March 18, 2014, and times prior to, employees worked on the rig derrick during the drilling of an oil/gas well. In the following instances the use of the secondary emergency only means of egress (Geronimo System) was obstructed: a) The Geronimo device was mounted on the exterior of the windwall exposing the employee(s) to the hazards of falling to the ground or rig floor, or delayed egress during a rig emergency. b) The Geronimo landing area had a wood dunnage rack sitting in the exiting path exposing the employee to the hazard of striking the rack and the dunnage it contained.

29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times: Drilling rig derrick: On or about March 18, 2014, and times prior to, employees worked on the rig derrick during the drilling of an oil/gas well where an emergency egress system was provided. The secondary emergency egress system (Geronimo) ground anchorage installation was not adequate to prevent its movement during use. This exposed the descending employee to the hazards of falling descent device or striking the ground at a high speed.

This exposed those employees to the hazard of being struck by the falling pipe or tool in the following instances: a) Catwalk area: The racks on the north side of the catwalk. b) Northeast corner of the well site: The tubular tools/equipment racks, on their west end.
