29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: Rig #5. On or about February 27, 2018, the employer did not ensure that flame resistant clothing (FRC) was on all outermost layers of employee's clothing when working in areas where there was a potential for hydrocarbons to be present, exposing the employees to fire and explosion hazards.
