Distances shall be measured from the live parts, if they are exposed, or from the enclosure front or opening, if they are enclosed. a) Storage Room - On or about 3/18/2014, employees were not able to access the electrical panel properly. The electrical panel was blocked by equipment and supplies. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1918.22(b): Each side of the gangway, and the turntable if used, did not have a hand rail with a minimum height of 33 inches (0.84 m)(measured perpendicularly from rail to walking surface at the stanchion) and a midrail. a) Dock - On or about 3/18/2014, employees were exposed to falling hazards when gangway did not consist of proper guardrails. The top rail measured 24 inches and did not have a midrail.
