29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: Employees engaged in manufacturing activities using the Cincinnati mechanical press brake, tooled and set up as punch, in the Culvert shop were exposed to struck-by and caught-between hazards in that the two hand controls were not configured as to require concurrent pressure from both hands during the closing portion of the stroke.
