29 CFR 1910.28(b)(1)(i): Each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was not protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:    Employees engaged in manufacturing activities at the west end of the North Pit in Building A were exposed to fall hazards of approximately 4 foot 5 inches from the walking working surface to the ground below in that the guardrail system was not contiguous and included open gaps spanning approximately 24 inches on each side of the rivet machine.

29 CFR 1910.29(b)(1): The top edge height of top rails, or equivalent guardrail system members, were not 42 inches, plus or minus 3 inches, above the walking-working surface:  Employees accessing the supervisor's office in the southwest corner of Building B were exposed to fall hazards of approximately 8 feet from the platform to the ground below in that the top edge of the guardrail system was found to be approximately 37.5 inches above the walking surface.

29 CFR 1910.29(b)(2)(i): Mid-rails were not installed at a height midway between the top edge of the guardrail system and the walking-working surface:  Employees engaged in manufacturing activities in the following locations were exposed to fall hazards in that:  a)  At the North Pit in Building A, employees engaged in manufacturing activities were exposed to fall hazards of approximately 4 foot 5 inches from the walking working surface to the ground below in that the mid-rail of the guardrail system surrounding the North Pit was not installed half way between the top edge of the top rail and the walking working surface.  The total guardrail height was found to be approximately 42 inches.  The opening between the walking working surface and mid-rail was found to be approximately 25.5 inches.          b)  At the work platform leading to the supervisors office in the Southwest corner of Building B, employees accessing the supervisors office were exposed to fall hazards of approximately 8 feet from the work platform to the ground below in that the guardrail system was not equipped with a mid-rail installed half way between the top rail and the walking working surface.  The total guardrail height was found to be approximately 47.5 inches.  The opening between the walking working surface and the bottom edge of the top rail was found to be approximately 46 inches.

29 CFR 1910.29(b)(3): Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds applied in a downward or outward direction within 2 inches of the top edge, at any point along the top rail.  Employees engaged in manufacturing activities at the north and south edges of the North Pit in Building A were exposed to fall hazards of approximately 4 feet 5 inches from the walking working surface to the bottom of the North Pit in that the guardrail system along the north and south edges were not capable of withstanding at least 200 pounds of force without failure.

29 CFR 1910.29(f)(4): Openings in a stair rail system exceeded 19 inches at its least dimension:  Employees accessing the supervisor's office in the southwest corner of Building B were exposed to fall hazards of up to approximately 8 feet from the walking surface to the ground below in that the stair rail system, installed prior to January 17, 2017, meeting the pre-January 17, 2017 top rail height requirements for a handrail/stair rail combination, was not equipped with a mid rail.  The opening between the walking surface and the top rail was found to be approximately 37.5 inches.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:  Employees engaged in manufacturing activities, using compressed air to clean out tooling, at the Trumatic 500R Trumpf CNC Punching machine were exposed to struck-by hazards in that the compressed air was not reduced to less than 30 p.s.i.  The compressed air was found to be at approximately 80 p.s.i.
