29 CFR 1904.29(b)(1): What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.  Located at Keihin Thermal Technology of America in Mount Sterling, Ohio:  a.  On the 2013 OSHA 300 log, the employer did not include all of the information that was required in Column F.    1.  Case number 13066, date of injury 10/09/13, does not include the cause of the strain to the right forearm.    2.  Case number 13067, date of injury 10/11/13, does not include the cause of the contact dermatitis, nor does it indicate the specific body part affected.  3.  Case number 13073, date of injury 11/15/13, does not include the cause of the contact dermatitis, nor does it indicate the specific body part affected.  4.  Case number 13078, date of injury 12/10/13, does not include the cause of the fractured right kneecap.  5.  Case number 13080, date of injury 12/11/13, does not include the cause of the fracture to the 2nd metatarsal of the right foot.    6.  Case number 13084, date of injury 12/03/13, does not include the cause of the contact dermatitis, nor does it indicate the specific body part affected.   b.  On the 2014 OSHA 300 log, the employer did not include all of the information that was required in Column F.    1.  Case number 14001, date of injury 01/03/14, does not include the cause of the lumbosacral strain.  2.  Case number 14003, date of injury 01/15/14, does not include the cause of the contusion to the right knee.    3.  Case number 14005,  date of injury 01/22/14, does not include the cause of the contact dermatitis, nor does it indicate the specific body part affected.  4. Case number 14006, date of injury 01/23/14, does not include the cause of the thoracic-lumbar strain.  5. Case number 14007, date of injury 01/23/14, does not include the cause of the sprain to the left wrist and the contusion to the left knee.   6. Case number 14027, date of injury 03/07/14, does not include the cause of the sprain to the right hip.    7. Case number 14028, date of injury 03/07/14, does not include the cause of the sprain to the right knee.
