29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) The employer failed to ensure employees were protected from falls while conducting roofing activities on a residential roof. The employees were installing roofing felt paper and shingles without fall protection. The employees were exposed to a fall hazard of approximately 13.4 feet. The AD Roofing L.L.C. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 313529703, citation number 1, item number 1 and was affirmed as a final order on September 22, 2010, with respect to a workplace located at 2938 N. Summit Ave., Milwaukee, WI 53211. The AD Roofing L.L.C. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 108047, citation number 2, item number 1 and was affirmed as a final order on December 21, 2011, with respect to a workplace located at W148N10254 Wingson Circle, Germantown, WI 53022.

The extension ladder did not extend at least three feet above the landing exposing employees to a fall hazard.
