29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one half hour:  (a)  South side of east building:  On March 20, 2014 and at times prior to; the employer exposed employees to fire hazards by storing 4 compressed gas oxygen cylinders and 2 acetylene gas cylinders side by side in the storage racks.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:  (a)  Along the north wall of the compressor room:  On March 20, 2014 and at times prior to; the employer failed to provide a cover for the electrical circuits of the champion compressor, exposing employees working around the compressor to an electrical shock hazard of 480VAC.  (b)  Along the west wall of the fabrication shop:  On March 20, 2014 and at times prior to the employer failed to cover a hole left open by a missing circuit breaker exposing employees working in the to fire hazards.
