29 CFR 1926.502(d)(15)(I): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and/or were not capable of supporting at least 5,000 pounds (22.2 kN) per employee: a) Duggan & Marcon Inc. - On or about March 21, 2014, employees on a flat roof more than 6 feet above a lower level were not protected from falling through the use of properly installed anchorage points to be used with their personal fall arrest equipment as part of a complete system.
