29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (0.9 m) above the upper landing surface to which the ladder was used to gain access: Employees were exposed to fall hazards because: a. the aluminum ladder located in front of the garage of the residence did not extend three feet above the roof. Employees were exposed to a fall hazard of approximately 11.4 feet above the concrete surface below. b. the aluminum ladder located on the side of the entrance to the residence did not extend three feet above roof. Employees were exposed to a fall hazard of approximately 12.8 feet above the ground.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards: An employee was exposed to fall hazards because: a. the aluminum ladder located in front of the garage of the residence did not extend three feet above the roof and the employer had not provided training for each employee that would have enabled them to recognize the hazards and provide them with procedures to follow to minimize theses hazards. Employees were exposed to a fall hazard of approximately 11.4 feet above the concrete surface below. b. the aluminum ladder located on the side of the entrance to the residence did not extend three feet above the roof and the employer had not provided training for each employee that would have enabled them to recognize the hazards and provide them with the procedures to follow to minimize these hazards. Employees were exposed to a fall hazard of approximately 12.8 feet above the ground below.
