Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards:     (a)  Cade Drilling, LLC at State Antelope 24-12-12 HNC, Briggsdale, CO 80514:  On March 15, 2014 an employee sustained a broken leg while being hoisted by a Rotzler Titan TH3 hydraulic winch approximately 30 feet above the derrick floor.  The manufacturer states that the winch is not designed, nor intended to lift or move personnel.  This condition exposed employees to falls and crushing hazards while being lifted by the winch to perform work at elevation.      Feasible and acceptable abatement methods include, but are not limited to, utilizing machinery and equipment that is specifically designed by the manufacture to lift personnel.

Section 5(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards:     (a)  Cade Drilling, LLC at State Antelope 24-12-12 HNC, Briggsdale, CO 80514:  On or before March 20, 2014 the back-up alarm on the Volvo L70E front-end loader was not functioning when the equipment was operated in reverse without a spotter.  The manufacturer of the equipment requires the reverse alarm to be operative when the equipment travels in reverse.  The front end loader is used to remove cuttings onsite.  This condition exposed employees to being struck by the equipment.        Feasible and acceptable abatement methods include, but are not limited to:      1) Ensure the front-end loader is operated in accordance with the operators manual regarding safe operation of the equipment.        2) Adhere to the lubrication and service chart located in the operators manual for the Volvo L70E.  A daily check of the reversing alarm is required.
