OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation hazards: a) Cade Drilling, LLC, Rig 24 at Anschutz Equas Farms 4-62-32-5764 CSB, Kersey, CO 80644: On or before November 10, 2017, the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation hazards. Employees used a Rotzler Titan TH3 hydraulic winch, Model # TH3CO.O, serial number 284116, for lifting employees when performing maintenance and when rigging down. This hydraulic winch was not designed nor approved for use in lifting employees. In the process of hoisting up an employee into the rig derrick, the employee caught his foot between two derrick braces, and couldn't shut the winch off in time. The hoist pulled off the employee's boot, and the employee experienced an amputation of part of his foot. The use of this hydraulic winch when hoisting employees in close proximity to the rig derrick exposed employees to amputation hazards. AMONG OTHER METHODS, SOME FEASIBLE AND ACCEPTABLE METHODS TO CORRECT THIS HAZARD INCLUDE: 1) Follow the manufacturer's directions for the Rotzler Titan TH3 winch, which provide that Rotzler Titan winches are not designed, nor intended, to lift or move personnel; and 2) Use other safe means for elevating personnel into the rig derrick, such as using the fixed ladder, or by using machinery that is specifically designed and approved to safely lift personnel.
