29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.    a.     On the second level east circular staircase, the wire cable around the floor hole had openings which measured between 7 inches to 11 inches thereby exposing employees to a 13 foot fall hazard to the floor below.    b.      On the second level west circular staircase, the wire cable around the floor hole opening which measured between 10 inches to 11 inches thereby exposing employees to a 13 foot fall hazard.
