29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:    a)    337 South 20th Street, Philadelphia, PA- Employees were exposed to fall hazards while performing sill replacement and pointing operations at heights up to 27 feet above a lower level from a fabricated frame scaffold without any form of fall protection.  Observed on or about March 21, 2014.

29 CFR 1926.451(b)(1): 29 CFR 1926.450(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports:   a)    337 South 20th Street, Philadelphia, PA- Employees were exposed to fall hazards while performing sill replacement and pointing operations at heights up to 27 feet above the lower level in that the scaffold levels were not fully planked. The five working levels consisted of one, two or three nominal 2x10 planks.  Observed on or about March 21, 2014.

Observed on or about March 21, 2014.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:  a)    337 South 20th Street, Philadelphia, PA- Employees erected a steel frame mason's walk through scaffold four frames high and began sill replacement and pointing operations at heights up to 27 feet above a lower level from the incomplete scaffold.  There was no one competent in scaffold erection present directing the work. Noted on or about March 21, 2014.
