29 CFR 1910.24(e): The fixed stairs were not installed at angles to the horizontal between 30 and 50 degrees as outlined, but not limited to the combinations given in Table D-1. a. At the maintenance shop, employees were exposed to a fall hazard, in that, a fixed stairway used daily to access the roof during refrigeration equipment inspections and testing was installed at a 76 degree angle.

Delivery truck hook up area, employees were exposed to an electrical hazard, in that, the 220 volt charging station number 329 had the outer insulation pulled from the plug thus allowing pull to be transmitted to the terminal screws of the plug.
