29 CFR 1910.147(c)(7)(i):  The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:    (a)  Employees cleaning, setting up and unjamming the Combi Packaging Machine (taping machine), were not adequately trained on the energy control program, in that they did not isolate and lock out the electric and pneumatic energy sources.      (b)  Operators and helpers setting up the Bobst Die Cutting machines, were not adequately trained on the energy control program, in that they did not isolate and lock out the electric, pneumatic, hydraulic and gravity energy sources.        (c)  Operators cleaning, conducting changeovers, and assisting with preventative maintenance on the Kohmann Window Machines #38 & #39, were not adequately trained on the energy control program, in that they did not isolate and lock out the electric and pneumatic energy sources.      (d)  Employees changed the wrap on the ITW Mima Stretch Wrapper were not adequately trained on the energy control program, in that they did not isolate and lock out the electric and pneumatic energy sources.       In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets:      (a)  Employees working on and around the Combi Packaging Machine (taping machine)  were exposed to ingoing nip points created by inadequately guarded conveyor chains and sprockets (case sealer guide chain) located just inside the case sealer exit on the taping machine, in that the chains and sprockets were not enclosed.          In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section:      (a)  There was no energy isolating procedure for the Combi Packaging Machine (taping machine).       Burrows Paper Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 312368780, citation number 01, item number 003 and was affirmed as a final order on April 10, 2010, with respect to a workplace located on Lyonsdale Road, Lyons Falls, NY 13368.           In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.147(c)(6)(i):  The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed:    (a)  The periodic inspection of the energy control procedures did not include a review between the inspector and each authorized employee, of that employees responsibilities under the energy control program, including observing the authorized employees actually performing servicing, setups, and/or maintenance activities on machines or equipment, such as, but not limited to, the Combi Packaging Machine (taping machine), the Bobst Die Cutting machines,  the Kohmann Window Machines #38 & #39, and the ITW Mima Stretch Wrapper.      Burrows Paper Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 314592643, citation number 01, item number 003 and was affirmed as a final order on May 19, 2011, with respect to a workplace located at 101 Commerce Drive, Mount Vernon, OH 43050.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
