29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: Employees who performed servicing on and around the Print Deck #8 on the Fischer & Krecke (F&K) Printing Press, were not required to: (1) turn off and shut down the machine, (2) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (3) affix lockout or tagout devices to each energy isolating device. On or about April 11, 2016, a printer operator and printer helper on the Fischer & Krecke (F&K) Printing Press, climbed into the Print Deck #8 to tape a tear in the web, without first isolating the electric and pneumatic energy sources and then attaching lockout devices to the electrical and pneumatic energy isolating devices.

29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: (a) On or about April 11, 2016, Printer Operators and Printer Helpers on the Fischer & Krecke (F&K) Printing Press, were not adequately trained on the safe application, usage and removal of energy isolating devices, prior to climbing into the Print Deck #8 to tape a tear in the web. Burrows Paper Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 964348, Citation Number 1, Item Number 1 and was affirmed as a final order on August 8, 2014, with respect to a workplace located at 2000 Commerce Center Drive, Franklin, Ohio. Burrows Paper Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 992531, Citation Number 2, Item Number 2a and was affirmed as a final order on Feb 20, 2015, with respect to a workplace located at 2000 Commerce Center Drive, Franklin, Ohio.
