Personnel entered confined spaces that included, but were not limited to, the mill stand pit, the water meter pit, and the set pit.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: A.) Blackhawk Neff Inc. - On or about 3/19/14, the employees utilized a man basket attached on a forklift without approval from the forklift manufacturer. The forklifts which the man basket was used on included but was not limited to, various electric Yale forklifts. The man basket was used on the forklifts for activities that included, but were not limited to, changing light bulbs.
