A hazard assessment was not conducted for employees that performed work tasks including, but not limited to, pumping, handling, and/or the transferring of corrosive chemicals in the Ebner Niagara Room of the facility.

Blackhawk Neff, Inc., 1209 Butler Avenue, New Castle, PA, on or about March 19th, 2014, and times prior thereto: Company employees were potentially exposed to eye and/or face hazards during work tasks including, but not limited to, pumping, handling, and/or the transferring of corrosive chemicals during water treatment operations. Company employees were not provided with complete and effective personal protective equipment training in that, company employees did not utilize face protection at all times during work operations with corrosive chemicals.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a.) Blackhawk Neff, Inc., 1209 Butler Avenue, New Castle, PA, Ebner Niagara room, on or about March 19th, 2014, and times prior thereto: Company employees work with and around corrosive chemicals, including, but not limited to, Biosperse 515 Microbiocide and Biosperse 250 Microbiocide. The emergency eyewash/shower station in the Ebner Niagara room was not in compliance with ANSI 358.1-1998 for Emergency Eyewash and Shower Equipment in that: 1. Weekly inspections of the equipment were not conducted. The emergency eyewash unit was not activated weekly to verify proper operation.
