29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels: a) At the facility, on or about March 24, 2014 - The employer did not obtain a new audiogram annually for a Saw Operator who received a baseline audiogram on December 5, 2012. The Saw Operator was exposed to noise at a level of 76.10% of the allowable limit during a 350 minute sampling period on August 29, 2012. This exposure is equivalent to a sound level of 88.0 dBa for the dose and time of saw operator's exposure.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: a) At the facility, on or about March 24, 2014 - The employer did not have a written hazard communication program. Employees use propane torches to cut grating. All American Grating, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.1200(e)(1), which was contained in OSHA inspection number 591798, citation number 1, item number 2a and was affirmed as a final order on October 16, 2012, with respect to a workplace located at 3001 Grand Avenue, Pittsburgh, PA 15225.
