29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident: Facility: On or about 12 March 2014 there was a forklift accident that resulted in an employee striking a stack of pallets. Forklift driver did not receive refresher training immediately after accident. Abatement documentation or certification is not required for this citation. Item was corrected during inspection.
