29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    600 Legacy Place, Dedham, Ma.:    1)          The employer did not develop a lockout tagout policy for the minibowl operation.    2)           A worker clearing a jam of the ball cup of the ball conveying mechanism of a minibowl lane did not utilize lockout tagout procedures prior to pulling on the conveying chain to clear the jam.
