29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:  Main plant, molding lines 1-5 and sand system 1 production machines: on March 25, 2014, employees locking out machinery for cleaning and other servicing and setup tasks were exposed to amputation, caught-in and entanglement hazards in that the procedures were not inspected periodically (at least once per year).  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  (a) Southwest, pour deck ladle #1, electric hoist's flexible cable: on March 25, 2014, employees were exposed to electric shock, burns, and fire hazards where a flexible cable's outer jacket was damaged exposing live electrical conductors at approximately 6.5 ft. above the adjacent work platform.  (b) Northeast, grinder #14, overhead lighting fixture with GE linear fluorescent tube light bulbs: on March 25, 2014, lighted bulbs in lighting fixture were not guarded against accidental damage and contact where located below 8 ft. above the adjacent floor.   There is no abatement documentation or certification required for this item.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  (a) South, core room, 240V circuit breakers panel A-10-A-A-B 1: on March 26, 2014, safe access to the electrical panel was obstructed by a core boxes table.  (b) Northeast, grinder #14 D-Square disconnect switch: on March 26, 2014, safe access to the disconnect switch was obstructed by a grinder's dust collector.   There is no abatement certification or documentation required for this item.
