29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: The employer failed to utilize energy control procedures or provide effective means of protection for employees using production machinery. Employees were exposed to cuts, lacerations, fractures and amputations while servicing the wheelabrator. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. On January 6, 2015 an employee was injured while performing servicing tasks. The employee was unjamming a cast iron part from the wheelabrator when his left index finger was caught in the machine feed section. The employee suffered a partial amputation of his left index finger and the employer failed to report the amputation within the required 24 hours. Abatement documentation is not required for this item.
