On or about January 20, 2014, at the above addressed jobsite, an employee was exposed to a caught by hazard while working with a press brake (i.e. Wisconsin Machine Corp., Model 12FM150, Serial # 72304) without point of operation guarding and/or alternate measures which provide effective protection, and without its pedal being protected.   b.  On or about March 26, 2014, at the above addressed jobsite, two employees were exposed to a caught by hazard while working with a press brake (i.e. Wisconsin Machine Corp., Model 12FM150, Serial # 72304) without point of operation guarding and/or alternate measures which provide effective protection, and without its pedal being protected.   c.  On or about March 17, 2014, at the above addressed jobsite, an employee was exposed to a caught by hazard while working with a bandsaw (i.e. Bandsaw, Jet Model No. JBS-14MW) that had the unused portion of the blade unguarded.  d.  On or about March 26, 2014, at the above addressed jobsite, an employee was exposed to a caught by hazard while working with a milling machine (i.e. Enco, Model No. 105-1114) that had its point of operation unguarded.
