29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. On or about January 20, 2014, at the above addressed jobsite, an employee was exposed to a caught by hazard while working with a press brake (i.e. Wisconsin Machine Corp., Model 12FM150, Serial # 72304) without point of operation guarding and/or alternate measures which provide effective protection, and without its pedal being protected. b. On or about March 26, 2014, at the above addressed jobsite, two employees were exposed to a caught by hazard while working with a press brake (i.e. Wisconsin Machine Corp., Model 12FM150, Serial # 72304) without point of operation guarding and/or alternate measures which provide effective protection, and without its pedal being protected. c. On or about March 17, 2014, at the above addressed jobsite, an employee was exposed to a caught by hazard while working with a bandsaw (i.e. Bandsaw, Jet Model No. JBS-14MW) that had the unused portion of the blade unguarded. d. On or about March 26, 2014, at the above addressed jobsite, an employee was exposed to a caught by hazard while working with a milling machine (i.e. Enco, Model No. 105-1114) that had its point of operation unguarded.

29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate: a. On or about March 26, 2014, materials and equipment were stored over an office area without having load limits posted, exposing employees to a struck by hazard.
