On or about June 4, at the above addressed jobsite, an employee was exposed to a caught by hazard while working with a press brake (i.e. Wisconsin Machine Corp., Model 12FM150, Serial # 72304) without its pedal entirely covered to prevent unintended operation.     b.  On or about June 3, 2014, at the above addressed jobsite, an employee was exposed to a caught by hazard while working with a milling machine (i.e. Enco, Model No. 105-1114) that had its point of operation not guarded.
