29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. On or about June 4, at the above addressed jobsite, an employee was exposed to a caught by hazard while working with a press brake (i.e. Wisconsin Machine Corp., Model 12FM150, Serial # 72304) without its pedal entirely covered to prevent unintended operation. b. On or about June 3, 2014, at the above addressed jobsite, an employee was exposed to a caught by hazard while working with a milling machine (i.e. Enco, Model No. 105-1114) that had its point of operation not guarded.
