29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     a) 601 Lehigh Avenue, Union, NJ 07083 - Manufacturing/ Machine Shop:     The employer did not establish an Energy Control program that defines machine specific energy control procedures, training and periodic inspections for employees performing part change out, maintenance and/or servicing machines to prevent accidental energizing of the machines. Machinery including but not limited to the 3 HOMAG "edge banders", 4 Shodas, and Beam saws did not have written procedures for de-energizing, and isolating energy sources.     Condition noted on or about 3/26/2014.

29 CFR 1910.212(a)(3)(ii): Point of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone during the operating cycle:     a) 601 Lehigh Avenue, Union, NJ 07083 - Manufacturing/ Machine Shop:     Employees that operate the HOMAG Optimat KAL 310 "edge bander" were exposed to amputation hazards created by unguarded points of operation. The rear door of machine was not interlocked to prevent accidental  energization of the cutting blade while employees cleaned the rear of the machine.      Condition noted on or about 3/26/2014.
