29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    (a)  Throughout Facility - On or about March 26, 2014, employees are exposed to hazardous energy sources on equipment on which employees perform servicing and maintenance, including equipment or machine set up, to include but not limited to hydraulic brake press, CNC Lathes, CNC Saws, Horizontal and Vertical Band Saws, Water Jet, and Tube Laserthe employer's written energy control program did not include the specific step-by-step procedures to control hazardous energy sources, to include but not limited pneumatic, hydraulic, mechanical, and electrical.

29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following:    (a)  Throughout Facility - On or about March 26, 2014, employees who perform servicing and maintenance on equipment and machinery with hazardous energy sources were not provided with training to the authorized level, in that they were not trained in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.     (a)  Throughout Facility - On or about March 26, 2014, operators of equipment and machinery with hazardous energy sources were not provided with training to the affected level, in that they were not instructed in the purpose and use of the energy control procedure.     (c)  Throughout Facility - On or about March 26, 2014, all other employees whose work operations are or may be in an area where energy control procedures may be utilized, were not instructed about the procedure, and about the prohibition relating to attempts to restart or re-energize machines or equipment which are locked out or tagged out.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    (a) Production Floor - On or about March 26, 2014, employees were exposed to amputation type hazards created by the unguarded point of operation on the 60 Ton Toyokoki Hydraulic Press Brake.    (b) Production Floor - On or about March 26, 2014, employees were exposed to amputation type hazards created by the unguarded point of operation on the 125 Ton Toyokoki Hydraulic Press Brake.    (c) Production Floor - On or about March 26, 2014, employees were exposed to amputation type hazards created by the unguarded point of operation on the 175 Ton Cincinati Hydraulic Press Brake.    (d) Production Floor - On or about March 26, 2014, employees were exposed to amputation type hazards created by the unguarded and unused portion of the blade on the Behringer Horizontal Band Saw.
