29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form:    a) Painting Area: Employees were exposed to 1,6 hexamethylene diisocyanate homopolymer while painting windmill molds with Amercoat 450H and the employer did not evaluate the respiratory hazards in this area exposing employees to acute irritation and sensitization of the respiratory system, on or about December 16, 2015.    No abatement certification or documentation required.

29 CFR 1910.1200(h)(3): The employee training did not include the requirements of 29 CFR 1910.1200(h)(3)(i) through (h)(3)(iv):    a) Painting Area: Employees using Amercoat 450H paints to paint windmill molds were exposed to 1, 6  hexamethylene diisocyanate homopolymer and the employer did not provide the employees information on the methods and observations to detect the presence of hazardous chemicals in the work area, the health hazards of the chemicals in the work area, and the measures employees can take to protect himself from these hazards, on or about December 16, 2015.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    a) Facility-wide: Maintenance employees are required to repair machines and the Lockout-Tagout Program procedures were not reviewed on an annual basis, on or about December 16, 2015.     Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    a) Storage Area: An employee, Machine Operator, was required to perform lockout operations on the Kasto win A5.6 while replacing the saw blade and step-by-step procedures for the machine on how to control hazardous energy sources such as pneumatic, hydraulic, and electrical were not developed or documented, on or about December 16, 2015.    Bassett industries, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.147(c)(4)(ii), which was contained in OSHA inspection number 964899, citation number 1, item number 1a and was affirmed as a final order on June 6, 2014, with respect to a workplace located at 2119 Sanatoga Station Rd., Pottstown, PA 19464.

Bassett industries, Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1910.147(c)(7)(i), which was contained in OSHA inspection number 964899, citation number 1, item number 1b and was affirmed as a final order on June 6, 2014, with respect to a workplace located at 21 19 Sanatoga Station Rd., Pottstown, PA 19464.
