On or about March 26, 2014 at 100 Monarch Village Way, Stockbridge, GA: An employee using two Makita circular saws to cut lumber on site with the shoe wedged in the up position, was exposed to a cut and laceration hazard.

29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. On or about March 26, 2014 at 100 Monarch Village Way, Stockbridge, GA: An employee using two 12 AWG electrical cords inconjunction with two circular saws was exposed to an electrical hazard when the outer sheathing had pulled away exposing the conductors beneath.
