29 CFR 1915.112(b)(1)(i):  The employer did not ensure that wire rope and wire-rope slings had permanently affixed and legible identification markings as prescribed by the manufacturer indicating the recommended safe working load for the type(s) of hitch(es) used, the angle upon which it is based, and the number of legs if more than one:    a)  On or about March 26, 2014, chain slings used to lift metal plates for use with the press brake were missing identification markings.    b)  On or about March 26, 2014, a wire rope sling had an illegible identification marking.  This wire rope sling was used with the Manitowoc Crane, model number 3900, serial number 39596, to lift metal plates.    c)  On or about March 26, 2014, three wire rope slings were missing identification markings.  These wire rope slings were used with the Manitowoc Crane, model number 3900, serial number 39596, to lift metal plates.

29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck, the employer did not ensure that each operator had successfully completed the training required by paragraph (l):    a)  On or about January 13, 2014, at the dry dock, the employee operating the Skytrack 10054 forklift had not been trained.    b)  On or about March 26, 2014, in the yard, the employee operating the Pettibone Traverse Lift, Model 8044, serial number 4181, had not been trained.

29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2):    a)  On or about January 13, 2014, in the construction area of the shipyard, a guardrail system was not installed along the deck of the 30000 East Side Barge, approximately 8 feet 1 inch above the ground, exposing employees to fall hazards.    b)  On or about January 13, 2014, in the construction area of the shipyard, a guardrail system was not installed along the edge of the flat area of a double module, approximately 6 feet 2.5 inches above the ground, exposing employees to fall hazards.      c)  On or about March 26, 2014, in the construction area of the shipyard, a guardrail system was not installed along the deck of a barge being painted, approximately 15 feet above the ground, exposing employees to fall hazards.    Sterling Shipyard was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 905493, citation number 1, item number 5a and was affirmed as a final order on October 18, 2013, with respect to a workplace located at 906 Main Street, Port Neches, Texas 77651.

29 CFR 1915.132(d): Worn and/or frayed electric cables were not taken out of service:    a)  On or about January 13, 2014, in the yard, a worn and frayed extension cord was powering a portable hand grinder.    b)  On or about March 26, 2014, in the yard, two worn and frayed 480V cords were powering welding machines.    c)  On or about March 26, 2014, in the yard, a worn and frayed extension cord was powering portable tools.    d)  On or about March 26, 2014, in the yard, a worn and frayed extension cord was powering a Roybi base grinder, #AB0930, and an IK-12 Beetle Torch.      Sterling Shipyard was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 905493, citation number 2, item number 1 and was affirmed as a final order on October 18, 2013, with respect to a workplace located at 906 Main Street, Port Neches, Texas 77651.
