29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees: without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph.  Note: This paragraph does not apply to large area scaffolds where the employer has satisfied the following criteria: the ladder was placed against a structure which was not a part of the scaffold and the scaffold was secured against the sideways thrust:  a) Side Entrance Scaffold and Step Ladder Near Roof - An employee accessing the roof was exposed to a fall hazard of approximately thirteen (13) feet while stepping from an A-frame ladder stacked on a one (1) buck high scaffold, on or about March 27, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(16):  Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or be tagged with "Do Not Use" or similar language, and shall be withdrawn from service until repaired:  a) Step Ladder, Near Side Entrance - The portable A-frame ladder stacked on a tubular welded frame scaffold used to access the roof had broken components (spreaders) and not immediately marked in a manner that readily identified the defect, exposing employees to a fall hazard of approximately thirteen (13) feet while accessing the roof, on or about March 27, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
