29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees:  (a) 242 Eden Road Lancaster, PA -  Employees were working from a tubular welded frame scaffolding applying a finish coat the exterior of the building.  The scaffolding was missing side- rails and mid-rails creating a fall hazard for the employees of approximately 25 feet to the ground below, on or about March 31, 2014.  No abatement certification or documentation required.

29 CFR 1926.452(c)(2): Tubular welded frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which would have secured vertical members together laterally:  (a)  242 Eden Road Lancaster, PA - Employees performed work activities from a tubular welded frame supported scaffold.  Several sections of scaffolding were missing cross bracing, thereby exposing employees to a possible scaffolding collapse and a fall of approximately 25 feet to the ground below, on or about March 31, 2014.   No abatement certification or documentation required.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   (a) 242 Eden Road Lancaster, PA - An employee was utilizing a JLG 400-S aerial lift to perform work activities and was not protected/restrained inside the basket by attaching the lanyard to the equipment's anchor point, thereby exposing the employee to a possible ejection and fall hazard of approximately 20 feet, on or about March 31, 2014.  No abatement certification or documentation required.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, or uprights did not bear on base plates, mud sills or other adequate firm foundation:   (a)  242 Eden Road Lancaster, PA - Employees worked on a scaffolding approximately 25 feet high and 28 feet long.  The scaffolding legs did not bear on base plates, on or about March 31, 2014.   No abatement certification or documentation required.   The Tri-Valley Stone & Stucco Solutions, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.451(c)(2), which was contained in OSHA inspection number 312504319, citation number 2, item number 1 and was affirmed as a final order on July 31, 2009, with respect to a workplace located at 500 Centerville Road (Heritage Hotel) Lancaster , PA 17601.

29 CFR 1910.1200(h)(3)(iv):  The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:Construction Reference: 1926.59).  (a)  242 Eden Road Lancaster, PA - Employees use hazardous chemicals in the work place, such as but not limited to Dryvit Sandpebble 1321. The employer did not provide employees with training on the revised hazard communication standard (HCS 2012) label elements and the new format and type of information on each section of the safety data sheets (SDSs) to ensure that when employees begin to see HCS 2012 compliant labels and SDSs in their workplace, employees understand how to use the labels and SDSs and access the information effectively, on or about March 31, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
