29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks; On or about March 26, 2014 and at times prior thereto, in the maintenance shop, employees using a Dayton model W1680 drill press operating at 1725 RPM without a guard over rotating parts were exposed to a caught in hazard.

29 CFR 1910.215(a)(4): Grinding machinery was not used with work rest(s) to support offhand grinding work: On or about March 26, 2014 and at times prior thereto, in the maintenance area, employees operating a Dayton model 2LKR9 bench grinder with 8 inch diameter wheel operating at 3450 RPM without a work rest were exposing to finger cuts, abrasions and caught in hazards.
