29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or before March 27, 2014 a Haas CNC machine was being operated with damafged doors that would not close completely and a hole in the Plexiglas type enclosure exposing the operator to some spray from the coolant used to cool the metal that was being fabricated as well as possible metal chips from the metal.
