29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a). On or about 10/20/15, on site, for employees that use hand held grinders on a daily basis. The employer did not provide flame retardant clothing to new employees. Company has policy to not provide flame retardant clothing until the employee has been employed for more than 30 days.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): a). On or about 10/20/15, on site, for employees grinding metal pieces with hand held grinder. Sparks are generated through grinding tasks and the employer did not require the use of leather gloves when grinding.
