29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  a) The employer failed to provide documentation and certification for abatement of citation 1 item 3 in which employees not wearing fall protection, were exposed to fall hazards in excess of 12 feet 7 inches to the ground surface below when performing framing, sheathing and other related activities at unprotected edges and while walking on the top plates of upper floors and walls of a new residential building.  These employees did not receive any fall protection training from the company.  Condition observed on or about 06/05/14.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:  a- T - 20 Grandstand Way, Cherry Hill NJ: Employees, exposed to electrical shock, were actively utilizing extension cords whose outer covering were visibly damaged in several areas.  These cords were located and being used throughout the worksite on the day of the worksite inspection.  Condition observed on or about 3/27/14.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided:  a- T - 20 Grandstand Way, Cherry Hill NJ: Employees, not wearing fall protection, were exposed to fall hazards in excess of 14 feet to the ground surface below when performing framing, sheathing and other related activities at unprotected edges of an upper floor of a new residential building.  Condition observed on or about 03/27/14.   b- T - 20 Grandstand Way, Cherry Hill NJ: Employees, not wearing fall protection, was exposed to fall hazards in excess of 12 feet 7 inches to the ground surface below when one was straddling and the other was walking the top plate of a new residential building's walls. Condition observed on or about 03/27/14.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a- T - 20 Grandstand Way, Cherry Hill NJ: Employees, not wearing fall protection, were exposed to fall hazards in excess of 12 feet 7 inches to the ground surface below when performing framing, sheathing and other related activities at unprotected edges and while walking on the top plates of upper floors and walls of a new residential building.  These employees did not receive any fall protection training from the company.  Condition observed on or about 03/27/14.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a- T - 20 Grandstand Way, Cherry Hill NJ: Employees, not wearing fall protection, were exposed to fall hazards iof approximately 12 feet 8 inches to the ground surface below when acessing and egressing a portable fiberglass extension ladder whose side rails extended approximately 10 inches above the upper landing surface.  This ladder was the only access and egress to that section of the building under construction.  Condition observed on or about 03/27/14.
