29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  a) The employer failed to provide documentation and certification for abetment citation 1 item 1a in which the employer did not initiate a program for a competent person to conduct frequent and regular inspections of the jobsite, to identify hazards and/or stop the work in the event workers were placed in a hazardous condition. As such, AMA Construction employees installing roof trusses on a 3 story new construction town home style building without any form of fall protection were exposed to a fall hazard of approximately 30 feet from the 3rd floor top plate to the ground.   The employees were using an aluminum extension ladder to access the upper levels of the structure.  The ladder had damaged rungs and was not properly set up .  Additionally, there was not a properly qualified rigger for the truss hoisting operations.  Observed on or about 6/05/2014.  b) The employer failed to provide documentation and certification for abetment citation 1 item 1b in which  AMA Construction employees installing roof trusses on a 3 story new construction town home style building without any form of fall protection were exposed to a fall hazard of approximately 30 feet from the 3rd floor top plate to the ground.   The employer did not provide training for exposed employees to recognize the hazards and the procedures to minimize fall hazards.  Observed on or about 6/05/2014.   c) The employer failed to provide documentation and certification for abetment citation 1 item 2 in which AMA Construction employees installing roof trusses on a 3 story new construction town home style building without any form of fall protection were exposed to a fall hazard of approximately 30 feet from the 3rd floor top plate to the ground.  Observed on or about 5/15/2014.            d) The employer failed to provide documentation and certification for abetment citation 1 item 3b in which AMA Construction employees installing roof trusses on a 3 story new construction town home style building were using an aluminum extension ladder to access the upper levels of the structure.  The ladder side rails did not extend at least 3 feet above the upper landing surface and the ladder had cracked and bent rungs.  The ladder had not been inspected for defects by a competent person.  Observed on or about 6/05/2014.

The ladder had visible damage to its rungs and its side rails did not extend at least 36 inches above the landing surface.  The employer did not provide training for employees to recognize and minimize the hazards.  Observed on or about 6/05/2014.       b) The employer failed to provide documentation and certification for abatement citation 1 item 3a in which AMA Construction employees installing roof trusses on a 3 story new construction town home style building were using an aluminum extension ladder to access the upper levels of the structure.  The ladder side rails did not extend at least 3 feet above the upper landing surface.
