29 CFR 1910.24(b): Fixed stairs were not provided for access from one structure level to another where operations necessitated regular travel between levels, and for access to operating platforms at any equipment which requires attention routinely during operations:    (a) At Missouri Cooperage Independent Stave Company on or about March 27, 2014 workers climbed onto stack of wood staves and then onto a platform to clean clogs from a stave jointer wheel.      NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    (a) At Missouri Cooperage Independent Stave Company on or about March 27, 2014 had a guard near the point of operation for a shaper head, but the guard was not designed such that it prevented employees from getting their hand or other body part in the danger zone.    (b) At Missouri Cooperage Independent Stave Company on or about March 27, 2014 the point of operation for all but one of the barrel ring riveting machines were not guarded to prevent injury.      NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.304(c)(4): Outside conductors rated 600 volts, nominal, or less did not have a clearance of at least 8 feet from the highest point of roofs over which they pass. The vertical clearance above the roof level was not maintained for a distance not less than 914 mm (3.0 feet) in all directions:    (a) At Missouri Cooperage Independent Stave Company on or about March 27, 2014 had the entry cable over a metal building's metal roof.  The roof was pitched less than 4 inches vertical to 12 inches horizontal and the cable came within 2 feet of the metal roof.      NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
