29 CFR 1910.23(e)(1): The railing did not consist of top rail, intermediate rail, and posts, and did not have a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level:  Employees working drilling rig #27, located at SEC 26, TWP 33, R 6, 400 fel, 400 fsl, Anthony, KS, were exposed to fall hazards from the mud mixing tank platform into the tank, in that the provided guardrails were inadequate:      a)The metal chains used for guardrails and mid rails sagged up to approximately 4 1/2 inches.   b) The end of one mid rail chain was not attached to the railing upright.

29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting as location requires:  Employees working rig #27, located at SEC, 26, TWP 33, R 6, 400, fel, 400 fsl, Anthony, KS, were exposed to caught in hazards of rotating horizontal shafting located approximately 15 inches from the floor.  Three Caterpillar diesel engines had approximately 5 inches of the 8 inch diameter shafts, operating at approximately 1200 rpm, located 15 inches back from the casings that were not fully enclosed.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  Drilling rig #27, located at SEC 26, TWP 33, R 6, 400 fel, 400 fsl, Anthony, KS; the working space in front of the 120 volt breaker panel for lighting, located in the electrical room was not maintained.  A battery charger and a shop vac were stored within the working space.
