29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part:       Employees working on an the roof, approximately 9 foot high, with a pitch of 4 in 12 roof slope, were exposed to fall hazards and the employer did not initiate and maintain an accident prevention program to control and eliminate the hazards present at the jobsite in accordance with OSHA regulations.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):     Employees conducting construction work such as but not limited installing roofing materials were exposed to fall hazards associated with an approximately 9 feet, on a low sloped roof of 4 in 12 pitch.   The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person to ensure employees were not performing unsafe operations at the jobsite.

1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:       Employees installing roofing materials while on a 4 in 12 pitch roof which was approximately 9 foot above a lower level, were exposed to fall  hazards.  They were not protected with the use of but not limited to using Personal Fall Arrest System, scaffolds, safety nets or guard rails for fall protection.
