29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) DeLine Box Company at 3700 Lima Street, Denver, CO 80239: On and before 3/10/14, the employer did not protect employees from hazards created by operating an unguarded portion of a Langstan Saturn III 50 inch model flexo folder gluer. This condition exposed employees to an amputation hazard of the roller conveyor. b) DeLine Box Company at 3700 Lima Street, Denver, CO 80239: On and before 3/10/14, the employer did not protect employees from hazards created by operating an unguarded portion of a Langstan Saturn III 50 inch model flexo folder gluer. This condition exposed employees to an amputation hazard of the rollers on the folding section. 29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the following means: (a) DeLine Box Company at 3700 Lima Street, Denver, CO 80239: On and before 3/10/14, the electrical connection for the Saturn III equipment had exposing live electrical connections to the foot pedal. This condition exposed the employees to the hazard of electrical shock and burns. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
