29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face. (Construction Reference 1926.103) a) Millersville University South Quad Student Housing - An employee sprayed closed cell foam insulation at the job site. The foam is created when two chemical parts are mixed in the spray apparatus. The part A chemical, Demilec A-PMDI contains Isocyanates. The employee who sprayed the closed foam product was not clean shaven and the facial hair came between the sealing surface of the facepiece and the face on or about March 27, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.134(i)(5)(iv): The employer did not ensure that the person authorized to perform the necessary sorbent bed and filter changes filled out a tag indicating by the signature and date that maintenance changes were performed: (Construction Reference 1926.103) (a) Residential Project - An employee sprayed closed cell foam insulation at a job site. The foam is created when two chemical parts are mixed in the spray apparatus. The part A chemical, Demilec A-PMDI contains Isocyanates. The employee who sprayed the closed cell foam product used an allegro breathing air pump. The date of the last filter change was not marked on a tag on the pump on or about March 26, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
