29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph:   a) On or about 3/31/14, on the first floor of the Sheridan Residence Apartment Complex under renovation, 2100 Sheridan Dr., Tonawanda, NY: The cord on a DeWalt Radio did not have a plug. Instead, the hot and neutral conductors were hanging out of the end of the cord, and were inserted into the hot & neutral slots at the female end of an extension cord. Furthermore, a drywall screw was inserted part way into the hot & neutral slots at the female end of the extension cord to keep the conductors from falling out. The drywall screw inserted into and sticking out of the hot slot of the receptacle was energized at 120 volts-ac.   b) On or about 3/31/14, on the second floor of the Sheridan Residence Apartment Complex under renovation, 2100 Sheridan Dr., Tonawanda, NY: A light socket in a string of temporary lights did not have a bulb in the socket and was not guarded. The socket was exposed & energized at 120 volts-ac.   c) On or about 3/31/14, on the second floor of the Sheridan Residence Apartment Complex under renovation, 2100 Sheridan Dr., Tonawanda, NY: A light socket in a string of temporary lights did not have a bulb in the socket. The socket did have a bulb guard, but the openings in the guard were very large. The socket was exposed & energized at 120 volts-ac.   NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.405(a)(2)(ii)(E): Lamps used in temporary wiring for general illumination were not protected from accidental contact or breakage:    a) On or about 3/31/14, on the second floor of the Sheridan Residence Apartment Complex under renovation, 2100 Sheridan Dr., Tonawanda, NY: One light socket in a string of temporary lights contained a florescent light bulb, but did not have a bulb guard. Another contained an incandesent light bulb, but only had one half of a bulb guard.    NO ABATEMENT CERTIFICATION REQUIRED

The plugs on the first and last extension cords in the series lacked a grounding prong.    NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:    a) On or about 3/31/14, on the first floor of the Sheridan Residence Apartment Complex under renovation, 2100 Sheridan Dr., Tonawanda, NY: A series of three extension cords supplied 120 volt- ac power to a DeWalt, Model DW367, 7 1/4" circular saw and a DeWalt radio. The outer sheath of the first extension cord in the series was frayed at the female end, exposing the conductors.    NO ABATEMENT CERTIFICATION REQUIRED
