29 CFR 1917.45(f)(4): Ladders, stairways, stanchions, grab irons, foot steps, or equivalent means were not provided on cranes or derricks to ensure safe access to walkways, cab platforms, crane cab and/or any portion of the superstructure that employees were required to reach. (a) On or about April 3, 2014 in the Warehouse Overhead Crane Access Area, the crane operators are required to traverse an open space of 25.5 inches across from the fixed ladder to the crane access platform. These employees are exposed to a fall hazard of approximately 20 feet.

29 CFR 1917.45(f)(5)(i): Glass in the cab of a crane or derrick was not safety plate glass or equivalent and good visibility through the glass was not maintained. (a) On or about April 4, 2013 in the Overhead Crane Cab, employees had wrapped the crane in non-transparent plastic in order to cut down on the wind and cold to which they were exposed. Employees would pull down the plastic so they could see; however, the visibility of the area was decreased exposing employees to being struck by loads and falls from loads which are inadvertently lifted with personnel on them.

29 CFR 1917.112(b)(1): Guardrails were not provided at locations where employees were exposed to floor or wall openings or waterside edges, including bridges or gangway-like structures leading to pilings or vessel mooring or berthing installations, which presented a hazard of falling more than 4 feet or into the water. (a) On or about April 3, 2014 at the Overhead Crane Platform, no guard was found to present on the platform where the crane operators were accessing the crane cab platform from/to the fixed ladder. This exposed employees to a fall hazard of 20 feet.
