On or about 03/20/2014, Crown Mountain Water., 13 C Estate Lindberg Bay, St Thomas - bottling area:  Employees worked and walked by two floor holes that were not protected by a cover exposing them to trips and fall hazards.   Note:  Certification of abatement must submitted for these citation items as required by 29 CFR 1903.19(c).
