29 CFR 1910.136(a): The employer did not ensure that each affected employee uses protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, and where such employee's feet are exposed to electrical hazards:  a. On or about 03/20/14, at the bottling area, at Crown Mountain Water, St. Thomas, VI. The employer did not ensure to provide protective footwear when employees handled 5-gallon (42 lbs) plastic containers during water filling, storage and delivery operations.  NOTE:  Certification of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c).

On or about 03/20/14, at the bottling area, at Crown Mountain Water, St. Thomas, VI. An emergency eye wash station was not provided in the immediate area when employees used and/or handled Sanitation Solutions corrosive products such as Detergent 3000 (potassium hydroxide) and Sani 3000 (sodium hypochlorite) during 5-gallon cleaning and sanitizing operations.   NOTE:  Certification of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c).

The employer did not develop a written hazard communication program when employees used and/or handled Sanitation Solutions corrosive products such as Detergent 3000 (potassium hydroxide) and Sani 3000 (sodium hypochlorite) during 5-gallon plastic bottles cleaning and sanitizing operations.
