29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:  a)  For inspection number 965771, the employer LG Construction, LLC, did not provide the certification of corrective action worksheet for Citation 1 Item 1 which was due 7/5/14.  The citation is for failing to designate a competent person to inspect site for fall hazards.  Employees were exposed to fall hazards up to approximately 32 feet.   b)  For inspection number 965771, the employer LG Construction, LLC, did not provide the certification of corrective action worksheet for Citation 1 Item 2b which was due 7/5/14.The citation is for failing to provide fall protection training to employees who were exposed to fall hazards up to approximately 32 feet.

29 CFR 1926.20(b)(2):  The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  (a) Jobsite - on or about March 19, 2014 - the employer did not designate a competent person to inspect the site for apparent fall hazards.  Observed hazards involve falls from 9 feet four inches, 20 feet and 32 feet.

29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a) Third Story Floor - on or about March 19, 2014 - no fall protection was available for the employees that were performing wood framing installations.  Elevation of the third story floor to the ground was approximately 32 feet.  b) Second Story Floor, on or about March 19, 2014 - no cover was installed for the 40 inch by 16 inch opening at the top of the stairwell.  Elevation from the top of the second story stairwell to the first story floor was approximately 9 feet, 4 inches.
